- Rebate claims and receipts due by 1/31/18

- $400 Consumer Rebate on a Crown Jewel III or Regalia with LOSM, purchase made between 11/1/17 - 11/30/17
- Rebate claims and receipts due by 12/31/17
- Upon claim approval, a rebate approval confirmation will be sent to the consumer through the email provided on the form. NOTE: It is important to keep this email because it will include the specific access code(s) for the Visa gift card(s). This access code is required to activate and use the Visa gift card(s).
- The consumer is solely responsible for lost, damaged or misdirected mail or email.
- Missing, incomplete or incorrect information will delay processing and will void rebate offer, unless expressly prohibited by law. Retain a copy of all documents for your records.
- Upon receipt of a claim for the rebate, Baby Lock will confirm that the sale is one which matches the intent of this program and reserves the right to withhold issuing the rebate in situations that do not match its intent. All documentation becomes the property of Baby Lock / Designer's Gallery and cannot be returned.
- Void where prohibited or restricted by law.
- Allow 12 weeks from the time we receive all required information for the delivery of your rebate. If you do not receive your Visa Gift Card after 12 weeks of submission date, please contact support@babylockrebates.com.
